I took a look at your account and found it was suspended because you have a Reservation Form on your account asking for credit card information which is and has not been allowed. The reason why this may have just happened for you now is most likely because we just found your form.
We consider credit card and other related information to be "sensitive information" and your form currently is considered to be phishing which is a violation of our Terms of Usage as we do not allow jotform fields to be used in that way.
In order to collect payment information properly and continue using your jotforms you will need to remove those fields and replace it with a approved payment integration. If you are willing to comply then we can gladly reactivate your account but it is recommended to do this as soon as possible to avoid resuspension.
