I have uploaded a screenshot of the form I make every month with Adobe.
Every month we have a different number of agenda items so I am able to add and delete lines as needed.
I also need to change the date every month and the info in the form.
I would like to be ale to create a form with your jotform that does the same thing.
Can you help?

You can easily create the form in our form builder.
For the header, you can use our "Header" field in Form Tools.
For the Text, you can use our "Free Text HTML" field, labeled as "Text" in Form Tools.
Text Box and Text Area are also both in the Form Tools section.
If you want to use the same form in the next month, I would recommend cloning the original form so you will keep it as well as its data. Deleting a field will also delete the submission so cloning is the best way to do it.
