My form works properly on Android, but not on iPhone

So exactly what I stated above... I created a form and it's been working wonderfully on almost all of the platforms but iPhone... Is there something I need to set or add to make it function on an iPhone?

I took a look at your jotform and I see that it is embedded using js embed code. What I would suggest is to use iframe embed code since it works much better on iPhones than the js embed code.
