How do I make fields that are left blank show up in the pdf?

You can do that by clicking on the small gear icon on the top right of your View Submission page, and unchecking the Auto Hide Empty Fields option.

If you are having problems finding your View Submissions page, please go to My Forms page, select the form you wish to view and click on the Submission button at the top.

Alternatively, you can find it from the More dropdown on the right of the form.
