    Can submissions/records be compiled in a spreadsheet?

    2) Assuming records can be compiled in a spreadsheet, can we ensure that each records is kept associated with the file uploaded with its form? If so, how would/could this work?

    All the data and information in the submission should remain intact.

    Yes you can compile them in a spreadsheet, you can try integrating Google Spreadsheet to your form, here's a guide on that: http://www.jotform.com/help/228-How-to-Integrate-Form-with-Google-Spreadsheet. The download link for the file, in case you added an upload field, should also be in the spreadsheet.

    You can also try downloading an excel report of your submissions, here's how to do that: http://www.jotform.com/help/44-How-to-Export-Form-Data-to-Excel.

    I would advise that you make test submissions to explore more how the form builder and the form works.
