Form Link Not Working

    I have many of my clients stating that the form is not working.  The link is below;

    On Apple devices it works on Android it seems to be an issue.

     

    The message states "Form Not Found".

     

    Please help as I have sent this to close to 1k people and I am getting many messages for this.

     

    Someone please help. I am new to this. If you need any info. please let me know.
