One of my customers placed an order on my Meat Cuts form, and received 55 order confirmation e-mails. I also received 55 separate submissions from them. Let me be clear - it was the same order, but just submitted 55 times.
My customer assures me that he only clicked Submit once, but still got 55 e-mails back. I did thorough testing with this form (and others) and have never encountered such a strange issue.
I searched the Forum but didn't see this problem anywhere else. Any idea what might cause this? I am still waiting to hear back from the customer on: 1) what he specifically ordered, and 2) what browser/version he is using.

Quite odd, I have never seen such a reported glitch, I guess you are referring to the last form submission by "Patsy Keim".
I could not find anything in our records indicating that the notification was sent more than once. I do see that your email notification is missing the sender name:
Sender Name :Make sure you assign it to a field from your form where the user enters her name (usually the"Name" field -or similar).
Has this happen again? We would appreciate any information provided by your customer in order to try reproducing this scenario. Hope it was a glitch that happened only in that moment.
