    Add two addional fields for our Application Process and Rules along out Privacy Policy

    I'm currently working and creating our Model/Talent Application Form. And we need to place our Application Process and Privacy Policy.

    I have try the following tools: 

    * text box

    * text

    But, that's not what where look for. Is the a way to create a PDF or Read filed?


    In any case you help is requested...Thanks!

    Have you tried using either of these widgets:



    They are actually designed for this kind of fields in your form.

    Hello kaynneclosette,

    I am not sure if I have understood your question correctly. The widgets my colleague shared above should work for you to display the terms & condition/privacy policy.

    Do you actually want to display a PDF file when the checkbox is clicked?

    You may like to add "PDF Embedder" widget in your form and then add conditions show/hide the PDF based on the click of the check box filed of your privacy question. 

    Please take a look at the following cloned form and see if this is what you wanted to achieve: 


    You will have to add your PDF file URL in the widget. When the checkbox is checked, the PDF will be displayed automatically.
