How to set email notification inside of the form

El formulario que me llega al mail, tiene espacios en blanco que quite y que no aparecen en el formulario a contestar, se pueden quitar estos espacios para que me llegue al mail solo la información?
Ya que si me meto desde la bandeja de Jotform me aparece como digo (solo los datos que deje en el formulario)

Y se puede cambiar el color etc.. del formulario que me llega al correo?

And I noticed that you do not have Email Notification inside of your form. 

In order to receive emails when users submits your form you need to set Email Notification inside of your form. Inside of this article you can find out how to set Email Notification:

Ya lo puse, se puede cambiar esta imagen por una personalizada?  

I am not sure if I understood your question correctly.  But I assume that you want to change JotForm logo with your company logo. If that is the case you can do that in Notification or Autoresponder depending which are you using. Inside of settings for Notification/Autoresponder go to Compose Email window and simply replace JotForm logo with your company logo. Delete JotForm logo and insert image and paste image URL of your company logo.
