Clients signature is not showing correctly in submissions

I had clients sign tx plans in client signatures and it has not saved although I pressed submit, the signature has returned as my own, this is a very big problem----Help!

I have checked your account and I did not find any form on it. But I see that this form is connected to this thread http://www.jotform.com/42816163308958. Are you referring to this form? When I inspected this form I see that there is Smooth Signature widget inside of it. 

I have also cloned and tested that form and smooth signature is working correctly. And I did got my sing correctly inside of submissions. 

I also noticed that there is no Email notification inside of that form. If you want to receive Email Notification when user submits your form you need to set Email Notification. Inside of this article you can find out how to do it:
