Hi ipbr21054,
Thanks for contacting us.
I checked your form and it seems that your email notification content is in plain text and no format.
If you want to have a more formatted/styled email, you can edit your email content and add styles to it or you can use the standard JotForm email table.
To edit your email content, please follow the instructions below.
To use the standard JotForm email table, you need to add new email notifier. To add a new email notifier, please follow the instructions below.
If you need further assistance, please feel free to contact us again.
Regards.

The form in question WAS using the standard Jotform email table.
It has just started to send plain text emails.
I dont see on your instructions how to use the existing contents but have your standard form applied.
I need the same form number but with th email table applied.
Not sure why it has changed itself ?
Please apply it for me.

DO NOT MAKE ANY CHANGES TO THIS.
I HAVE NOW HAD TO DO SOMETHING ELSE AS TIME WAS NOT ON MY SIDE
PLEASE CREDIT MY ACCOUNT WITH SOME E-MAILS DUE TO MANY TESTING EMAILS SENDING TO MYSELF

Please do not write in all caps, the use of all caps is considered impolite and perceived as "shouting".
If you need the standard table then follow the instructions provided by Christian above. You will need to recreate the notifier so you can get the standard table with the logo and the table. I can see that you already created a new form and the notification is already configured properly with the standard table on it.
As a one time accommodation, I will reduce your submissions, from 37 to 27. There are 10 test submissions that you made today.
Moving forward, make sure to limit using your submissions for testing purposes, because it will affect the number of your submissions.
Let us know if you need any help. Thank you.

Capitals were intended to bring your attention to not do anything.
You need to put something in place for testing purposes without emails be used up.
The reason for the post was to say that the form was always using the standard table but then vanished.
