I created my form, got the embed link and place it in Storyline as a web object. It shows in preview but once published I get a blank screen.
I have tried Https and secure and it still does not work.

The code you referenced is simply the link to your form, not the embed code. To embed your form, you will need to use the full embed code:
The one that usually works best in most sites is the iFrame embed code:
If there is a way to insert HTML rather than a link in Storyline, that will probably be your best bet. From what I can tell, web objects in Storyline only accept links, not full embed code.
