We would like to terminate our account with JotForm, please assist us in doing so, or provide instructions on how I can do it manually.

I went ahead and manually cancelled your subscription with our payment processor to ensure that you would not be billed again. Your forms and account will remain active and usable as long as you do not go over your Free account limits.
