Adding username and password for forms?

I would like to create an order form for wholesale customers.  Since the prices on the form are wholesale and not retail is there a way to create a distinct user name and password I can give each customer to access and order from the form?

    Hi,

Unfortunately, we do note allow forms to have fields that ask for user names and passwords. This is because it mimics a phishing form. We have an anti-Phishing system that scans forms and autosuspends account that violates our Terms of use, please check it here: http://www.jotform.com/terms/

Rather than a username or password, you could try taking advantage of the "Coupon Code" in the payments wizard to set specific prices. Here's a guide for the coupon code: http://www.jotform.com/help/233-How-To-Add-Coupon-Code-in-the-Payment-Form.

Another workaround is that you change the field names to something that does not mimic or violates our terms. You can try naming it "Verification Code", "Unique Code", "Unique Key" or something that can be easily understood by your users.

    I hope that helps.
