Asked on May 21, 2015 at 2:23 PM

I have finished my two forms and now need to send, post, populate the data entered by a visitor to my second form.

this is the form a customer will use when visiting website.

    http://www.jotform.com//?formID=51386206687160 (FORM1)

When the customer presses Submit, form2 should be pre-populated with the answers from form1.

    http://form.jotform.us/form/51386186795167  (FORM2)

    Thank you,

    Bill Talero

    Hi Bill,

I suggest reviewing this user guide:

    -Prepopulating-fields-to-your-JotForm-via-URL-parameters

To achieve this on your jotform http://www.jotform.us/form/51386206687160

use this code in the custom URL Thank You page of the form

    http://www.jotform.us/form/51386186795167?firstName9={firstName}&lastName={lastName}&address={address}&email={email7}&address6={address6}&state={state}&zipCode={zipCode}&phoneNumber[phone]={phone}

Make sure also that Send Post Data method is OFF

You can test submit my demo form http://www.jotformpro.com/form/51405924573961 also.

    Example output is like this:

No, when the person fills the data in the first form, he will be redirected to the direct link of the second form.

It does not work. i put the form on my website sleeptestnow.com and then tried to fill it out like a regular customer. You can try it yourself to see if it does the same.

    My two forms are

    Form1  http://www.jotform.com//?formID=51386206687160

    Form2  http://form.jotform.us/form/51386186795167

I still cannot get my form2 populated. I used the code above and then added more fields to pre-populate. When I complete form1 and press submit, I do get the form2 but all fields are blank.

    Please help.

Thank you for your kind assistance.

As of my colleague Jonathan above explained to you, he has elaborated a demo form link :

     http://www.jotformpro.com/form/51405924573961

I went ahead, tested said demo and works, the step two form is populated with fields already filled out, check this out and apply: http://www.jotform.com/help/71-Prepopulating-fields-to-your-JotForm-via-URL-parameters

If any other question please do contact us.

I don't understand. the demo above is for an earlier version of my form. The new version has new fields. So following Jonathan's example, I created the new thank you URL.

http://www.jotform.us/form/51386186795167?First Name={firstName}&Last Name={lastName}&E-mail={email7}Address={address5}&City={city}&State={state}&Zip Code={zipCode11}&Phone={phone}&Date of Birth={dateOf}&Gender=Gender}&1. Do you snore?={Snoring}&2. Is your snoring loud?={SnoreLoud}&3. Have you ever been told that you stop breathing, gasp or cough while you sleep?={Breathing}&4. Male neck or collar size?={MaleNeck}&5.Female neck or collar size?={FemaleNeck}&6. Have you ever been or are you currently being treated for high blood pressure?={HTN}&a) When you are not busy or active?={SleepyBusy}&b) When you are driving or stopped at a light?={SleepyDriving}&This is your Sleep Apnea Score(SAS)={SAS}&Height (ft)={heightft}&Height (inches)={heightinches}&Weight (pounds)={weightpounds}&Your BMI={BMI}&Current Medical Conditions? Please follow the following steps:

    1. Make sure the Radio Option buttons are not set as 'Read-Only' fields

The reason for "read only" option was to prevent anyone from changing any field once it has been populated - is the another way to accomplish this?

Unfortunately, that is a limit on the functionality of the Radio option.

A more sophisticated method to accomplish the same is to use pre-population via Send Post Data method.

    Check this user guide: -Send-POST-Data-From-JotForm-Using-PHP-in-Custom-Thank-You-Page

It will require though using PHP server-side setup and a bit of knowledge on PHP.

Hope this help. Let us know if you need further assistance.

    Thanks.

I did a bit more search on this..

Since the 2nd form are just being pre-populated anyway, and you do not want them to be editable  anymore, then why not just use regular Textboxes field as the destination field for the Radio options.

You can pre-populate the Textboxes even when they are at Read-Only mode.

Hope this help. Tell us your thought on this.

    Thanks.

    Thanks Jonathan, that worked perfectly.

I don't want the populated form to pop-up when the visitor completes it. Form2 is a medical prescription and needs to be emailed to the physician. When the visitor completes the form and submits it, the visitor should only see a simple thank you message and the second forms goes directly to the physician.

    Form2: http://www.jotform.com//?formID=51386186795167

    Kind Regards,

    Bill Talero
