Show headers and text and printing form with text boxes

the title and text boxes don't appear on the form once it is submitted

If I understand you correctly, you are referring to the submissions right?

By default, only the form fields and answers will show in the submissions area. However, you can show other texts and images from your form to appear in the submissions by enabling the "Show Headers and Texts" option. Please take note that text boxes will not appear in the submissions. If you want to print the form with the answers and text boxes together, then you need to include the {edit-link} in the email notifications.

Once you receive a an email notification, you can click the "Edit Submissions". It will direct you to the original form with the answers and text boxes. From there, you can print it from the browsers.
