I checked your site and I can't see the embed code/source code of your form in the site. May I ask how did you embed the form on your site so we can check?
I also recommend that you use the iFrame embed method. You can check this article to know How to Get the iFrame Code of your Form.

I'm afraid that you may not have correctly embedded your form. After looking at the source code of your website, I see that you are trying to use an ordinary script embed.
This script embed method is conflicting with the JQuery scripting library you have on your website. That is why my colleague suggested that you use the iFrame embed method, as the iFrame method will ensure that there are no conflicts between the scripts of your page and the scripts of the form.
