    Hi Jotform!

I am trying to create a form in jotform that i have already created in muse, unfortunately I cannot seem to get where I need to be.

Below is an image of what i am trying to create. I have tried the styler and themes also but cant see to get close.

the color is 2489c5 and needs 2px outline and the font is Lato. the size of boxes is 320 x 43

    the form is http://www.jotform.com//?formID=51446156973967

I am sure i can figure out the rollover states if you could get me started. sorry to be lame.

If anyone could give pointers i would be very grateful!

    Thank you!

If you are asking if your desired design is possible with JotForm, then yes, that is possible with the help of the Designer and Injecting Custom CSS to you form.

However, please be advised that the fields included in your form would violate JotForm's Terms of Use regarding Phishing.

-Do not create login forms (username or email fields along with password fields combined) in any forms.

Jotform is strict about this matter and has an Anti-Phishing System that automatically detects forms and suspends accounts that are suspicious and could be or is involved with phishing activity.

    Haha, I'm sorry!

The design was from a muse mockup and the fields I require are for an email newsletter so just name and email are required. I beleive it was shown in the jotform form link posted accompanying the post.

I am aware of not using jotform for logins. why would I? I cant think of anything less secure.

Anyways, hope that puts your mind at rest,

Please take a look at this cloned version of your form: http://form.jotformpro.com/form/51446929558974. Is this what you want to accomplish?

I created this form using the form designer to emulate the colors and layout on the provided screenshot.

And added the CSS code below to line up the submit button.

Feel free to clone the form to your account if you want to inspect it further.
