I cannot get the email option to show up when I am on the "Match Form Fields" page in the integration wizard. The dropdown menu shows:

If I am correct, this is happening on your jotform http://www.jotform.us/form/51453972845161 within your romej14 account?
Based on what you have told me I believe that the issue is caused by your browser cache.
What I would suggest is to clear your browsers cache, which you can do by following these steps: 15 Ways to Clear Your Browser's Cache - wikiHow
I have also cleared your forms cache (just in case), so please do let us know how it goes.
