Is there a way to show the headers in the PDF format for a submission?

Would like to have headers in PDF document.

On your form's Submissions page, simply click on the small gear icon near the top right corner, and select the option to Show Headers and Text. Your PDF submissions will include headers and terms of use text from now on.
