I am keen to create a form with several sequential fields that collects data on vet drugs in unusual spoecies andf loads them into a database so we can ineterrogate the database by various taxonomies ( substance, species, indication, credability of reference for this data etc) and late from the database get a specific piece of dosing information back via a third party interrogation tool.

I am building a sample form in adobe forms  - but I may be wiser starting in Jotform with your helkp.... please can you be of assistance?

If you wish you can import the form from Adobe FormsCentral using our import tool:

    https://www.jotform.com/adobe-formscentral-alternative/

You can see our guide on how to create the 1st form:

From what you describe maybe creating a Grid Report listing might be useful as it allows you to filter according to keywords:

You can see here how to create a Grid Report:

    https://www.jotform.com/help/105-How-to-create-a-Grid-Listing-report

Let us know if we can be of further assistance.

Sorry, I forgot to mention, you are able to download the form responses in PDF, Excel and CSV format, by selecting My Forms > select the form > Submissions:

    Sorry.
