Source Code - Errors

If the form itself is working but the embedded copy is not, this usually means something in your page is interfering with your form.  Any scripts or CSS in your page will interact with your form and can cause some unintended error such as yours.

Try replacing the source code with the iFrame code to see if that remedies the errors:

If the form functions properly embedded using the iFrame code, then there is something in your page causing the error.  If the form still has the same display error while using the iFrame code, let us know and we will be happy to have another look.
