how do I take info from contact form and auto populate a pdf contract

I have created a contact form,

linked it to my website,

now I would like to know how to use the information collected to auto populate a pdf document or contract.

any direction would be helpful.

I have done some research regarding you question,

If you are using adobe you need to create a CSV file whose column name match the field names in you fill-able form.

The same steps apply for foxit PDF reader

so I have found a Conga Composer app that looks like it will be able to do what I was looking for.
