Conditionals should hide "Click to upload files" and show the camera capture widget.
Works on a PC - win8, chrome ok but not on iphone
I get a "choose file" button that can take me to the camera.....Is there a way to change the text to "Take Picture/Video"

I checked your page on an iPhone simulator and the condition appears to be working properly. Since I am using a simulator, rather than an actual iPhone, we may get different results:
Does the widget work in the form itself but not in the embedded version? If this is the case, try embedding your form using the iFrame method instead:
Replace your current embed code with the iFrame code to see if that helps.
If the widget does not work in the regular form or the embedded form, let us know that as well.
