Can I add background image to a template?

I modified one of the templates for a questionnaire I have for my clients and wanted to add a background image. I went to the design center but it would not allow changes. Can you please help?

Basically, using the Form Designer you can add the background image like this

Thanks so much for the reply. I modified the Instructor's Performance Evaluation template and it will not allow me to change the background like this. Here is the link:

I was trying to place the image on the bottom of the form as the background.
