Multi-file upload field does not work properly on iPad

I am having serious problems with multiple file uploads.  When someone uploads multiple pictures from an iphone, it keeps the first picture and then repeats the second picture.  So say it I upload 10 pictures...on the pdf report, it shows picture 1, the repeats picture 2 another 9 times, omitting the other 9.

If you try to upload from a PC or an android device, it only lets you select 1 picture(even though the form is enabled for multiple uploads.  

I have it selected so that they can do multiple file uploads and when I first installed this form it was uploading fine.  If it makes a difference it is embedded as a link in a webpage so I could populate a dropdown dynamically, though I don't really see how that part would be related.

I took a look at the form you have given a link for above, but it seems to be utilizing the single file upload field instead of multiple file one.

Was this like that before or is this intentionally changed?
