Dear Sir or Madam,
We are testing a form and altough it has worked for a few weeks, it is not possible to see it as it says "Form not available". Would you please be able to confirm it this is a server problem from Jotform end?
We have not modified the form recently at all.

I've checked your forms and saw that you have a form named ''Untitled Form'' which looks like a phishing form. You can't create a form like this within JotForm. That's why your account has been suspended. After you change or delete this form, your account will be re-activated. We'll be waiting for your response.

Hello AtacanC I did not have a "untitled form" and beleive someone hacked into my account, which is very inconvenient.
Please AtacanC can you reenable my account and trigger a change of password on my account? This is horrible because we have lost all of our database of responses.
Please contact us at firstname.lastname@example.org as this is quite distressing. I agree with you the form has to be deleted (i have no idea how to make a phishing form - this is not me).
Please re-enable my account.
PS: My activity on this is totally genuine, and again someone MUSt have hacked my account as I certainly did not create a phishing form. In fact I did not create an "untitled form" at all so this "smells bad". Thank you for your help.

I've re-activated your account and whitelisted your forms. Please go to ''My Forms'' page and scroll down. You will see the Account Settings button. Click on it and you'll be able to change your password.

I am changing my password straight away. And as you will see i am NOT into dodggy business, apologies but this was not me.
