Sending forms

    I have created two forms. the first form is active on my website and visitors can complete the form as a normal submission. I have a second form that gets populated with the answers from the first form.

    The second form is for a physician to sign and forward to me. the form is a "prescription" and such has to be saved and kept for 7yrs, not just the field contents, but the entire form as you see it.

    My question is:

    Is there an app/widget that would allow me to send the form as a WYSIWYG or similar to the physician.

    Hi,

    If you are trying to send a pre-filled form to someone else, you can use the autofill app to create a URL that is filled specifically with your data: 

    http://prepopulate.jotform.io/

    You can then send them the pre-filled URL.  This will not save a copy of the form unless the form is submitted.

    You can also simply send them the edit link:

    http://www.jotform.com/help/40-How-to-Let-Users-Update-Their-Form-Submissions-at-a-Later-Date 

    Other than those options, you can get quite close to the actual form by including headers and text of your form in with your submission data:

    http://www.jotform.com/help/305-How-to-show-headers-or-text-for-contracts-terms-agreements-in-the-PDF-format-of-a-submission

    You can then include a PDF in your submission notification:
