Did you meant you cannot see your forms in your My Forms page?
I just checked your account/username drsamjinich
and noted that it is Active and in good standing.
I found 8 forms under the account.
Please try closing the browser first then relogin back to jotform http://www.jotform.com/login
then check if this resolve the issue.
Let us know if issue remains.
