Can entries from one form populate another?

Trying to create a phone app where my employees can select "client a, b or c" from a drop down, enter a time of activity/incident, then a narrative text box. When they submit that single entry form is there a way to have those single entries for clients a, b or c to compile into one "oldest entry to newest entry" report?  example:

EMPLOYEE:  JONES, TIM       DATE/TIME:  05/15/2015  16:45    CLIENT:  ABC PLUMBING

NARRATIVE:   CHECKED ALL AREAS RELATED TO WATER LEAKAGE. EVERYTHING FOUND IN GOOD ORDER

    SUBMIT

At the conclusion of the shift, all entries related to ABC PLUMBING would result in a single, chronological report, which could then auto or manual mail to the client.  Thanks!
