I see on your site that I still am under the free subscription. I got an email saying that I had paid and been
accepted for the upgrade. While I am still not seeing my customers forms that's applying to my site.
The page is still blank

Upon checking your account, I did not find any issue with that. It is already upgraded to premium monthly subscription.
If you are still not able to see your account in upgraded status, you may like to clear your browser cache and try again. The following guide should help you clear your browser cache: http://www.wikihow.com/Clear-Your-Browser's-Cache
