I received message saying quota has been reached. There have only been about 4 test submissions

    Hi Reid,

    First off, thanks for using JotForm. I've checked your account and it seem that you've created a guest account. You've also used SSL submissions. That's why your account has exceeded its allocated quota. You can register for free and get monthly 100 regular submissions or 10 SSL submissions whichever comes first. But you need to use the same email address, you used for your guest account, to create a free account on JotForm. So you can keep your existing forms.
