Only the direct link works but none of the embedded versions work. PLEASE HELP!

Did you remove the embedded code? I can see you have hyperlinked the direct link:
Please try embedding with the iFrame code: https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code
If the the form does not show, do not remove the embedded code, and let us know, so we can check further.

I did that but it still isn't showing up. I can see the form when I am editing my website, but once I preview it, the form disappears

I have replicated your website and cloned your form.
I have found out that you have placed a link of your form to your website and have not embedded it yet.
As what my colleague have mentioned you may use the iFrame code to embed your form to your website.

I have checked your website's code and found this:
It should look like this:
Then, kindly preview your form.
Let us know if you're still having the same issue so we may look into it further as I was able to embed a cloned copy of your form to a replicated version of your website successfully. Thank you.
