On my page, I'm looking to provide clients with a list with 15 options and ask the question...
What are the Top-5 areas that NEED THE GREATEST IMPROVEMENT?I'd like them to be able to select 5 options but then rate the priority of those 5 options(greatest need, 2nd greatest etc). The priority can be as easy as ranking each one on a 1-5 scale.
Any thoughts on how to accomplish this?

Hi,
There are a couple ways I can think of to accomplish this. The first would be fairly tedious, but you could have a checkbox with your 15 selections, then conditionally shown scale rating fields for each selection. In the end, you will have the 5 rating fields based on the 5 selections made in your checkbox field. Using manage multiple fields works great for copying multiple fields:

You would need 15 conditions set up like this:
One for each checkbox selection and rating pair. Duplicating the previous condition then changing the selections to the next one in the list makes this quicker than you would expect.
