The Create Form button is not active on my profile, so I'm unable to create forms from scratch. Also, I'm unable to create folders. Is this because I'm a sub-user? Thanks.

Please try to click one of the folder under "Shared With You". After clicking it, the "Create Form" button should be enabled.

Thanks! This works. Now how can I create folders. I'm going to be embarrassed if is this simple as well.

Please take note that this form that you will create will be under the Shared Folder. The reason you cannot create a form on your own account is because you are using a Sub user account. It means that you cannot create a folder as well.
