Hello. Could you please let us know the link to the page where you have embedded the form, so that we can take a closer look and assist you?
If it is not embedded anywhere, would you please let us know what form is this happening to?
I believe that it is caused by the embed method, that the height of the iFrame is not set to be high enough when on a narrow screen, but unfortunately we can't test this without seeing the web page.
We will await your kind response. Thank you

I also can see it working already on my side.
on Android browser
Please try refreshing your mobile browser also. Perhaps it is just a cache issue and it was giving a cache view on the browser only.
Let us know if still not resolve.
Thanks.

I've refreshed it many times.
You can also see at your mobile that question no. 5 is still cut off.
at my cell-phone question no. 3 is also still cut-off.

Sorry for the delays.
I confirm also that #5 is cut-off on the mobile browser.
It seems that the caused of issue was that item #5 is Configurable List widget.. so it is not being properly affected by the mobile responsive widget.
The only workaround I could find at this time is to adjust the question for the #5. That is, decreased the column field for the widget. Unfortunately I could not find any other way.
Let us know if this suggestion can work for you or not.
Thanks.
