We do not allow for the collection of usernames or passwords through our form. One of your forms asks for a Login Password:
Though you may be collecting it for good reason, we simply do not allow it to avoid confusion. Passwords give phishers direct access to accounts, to protect our users, we do not allow for the collection of them and our system is set to automatically suspend accounts that do so.
I removed the password field from your form and reactivated your account. You should now be able to use it once again.
