Form shows up blank when embedding to Facebook

the pop up link that allows me to ad the app on FB displays a blank box...please help!

The first thing you need before you can embed a form on Facebook is a page to embed it to:

https://www.facebook.com/help/174987089221178

After you have created a page, you can then add a tab to that page:

http://www.jotform.com/help/22-Adding-Form-to-Facebook-Pages

You may need to clear your browsers cache/cookies if it still does not show up.
