My form does not seem to be sending email notifications. Can someone take a look at it to see if something is misconfigured?

I checked your jotform http://www.jotform.us/form/51546649517161 and noted that it has a customize email notifier
the Notifier seems to be not updated anymore.
I suggest creating a new Notification Email for the form. using the Add New Email button in the Email Alert.

I changed nothing and it seems to be sending again. Will update if changes persist.
