Your form looks fine on my Firefox. (I am also on a mac)

Is it not working on your site? Can you post the URL of the page? Can you try a different form code such as "IFrame Code?

    On the Form Builder:
    1. Open "Setup & Share" tab on toolbar,
    2. Click on "Share Form"
    3. Click on "Advanced Options"
    4. Click on "IFrame Code".

We also have special instructions for iWeb Forms, have you tried that?
