My form does not work on mobile devices

    I have tried both the java script and iframe. On android when you fill out the form it simply does not submit a form and the screen goes blank. Same with some iphone devices. On ipad mini 2 the year field does not show and the month field will not except the month.

     

    Please advise.

    Upon taking a look at your website, I see that there is some mixed content - the page is loading non-secure content on a secure page, and this might be causing the issue.

    The form itself doesn't present this issue when accessed directly, so it seems that this non-secure content is loaded by your page.

    The content loaded over regular HTTP protocol seems to be related to the Google fonts that you are using:

    Can you please ensure that your website is using fully secure content, meaning that the fonts are loaded in a secure manner as well, and try again?

    Also, can you please check if the form behaves properly, and if you can correctly submit the form, when you are accessing it over its direct link? https://secure.jotform.us/form/51386385457163
