Where are the form submissions?

I've done a few tests and others have been using the form as well but there seems to be no record of the form submissions.

Is it the form Y-con Panels? I checked and there are 2 submissions on the My Forms and Submission area:

I checked the email notification and it is missing the sender name. This might cause the email provider to block it as spam.
