How to have Unique Question on the Form

    Hi i send my form to my Customer, but hes doesnt send the soumission(response) because they received a popup with mention " multiple response are not activate)

     

    how i fix that

    I just submitted a test without any problem with the 3 forms you have in your account.Is the form embedded in a website? If so, please share with us the URL of the page.

    yes is this form and i tried with 2 customer and is doesnt work

    Form Limits using Unique Question is activated on your jotform http://www.jotform.ca/form/50285665570258

    Check the user guide: -How-to-set-Form-Limits-Based-on-a-Unique-Question

     

    Disable/Remove the Unique Question set, will fix the issue of the pop-up message.
