    Hi there;

            Have you tried iFrame method. I suggest you to use iFrame instead of simple embed method. Please review the following screenshots on How you can embed your form using iFrame.

    I hope this helps. Please let us know further if you still have queries on this, we would be happy to help you.

    Thankyou,

    Hi,

          I checked the form and is cut-off from bottom, try to increase the height to 100% of iFrame.

    <iframe id="JotFormIFrame" onload="window.parent.scrollTo(0,0)" allowtransparency="true" src="//form.jotformpro.com/form/51534064530952" frameborder="0" style="width:100%; height:100%; border:none;" scrolling="no"></iframe>

    <script type="text/javascript">window.handleIFrameMessage = function(e) {var args = e.data.split(":");var iframe = document.getElementById("JotFormIFrame");if (!iframe)return;switch (args[0]) {case "scrollIntoView":iframe.scrollIntoView();break;case "setHeight":iframe.style.height = args[1] + "px";break;case "collapseErrorPage":if (iframe.clientHeight > window.innerHeight) {iframe.style.height = window.innerHeight + "px";}break;case "reloadPage":window.location.reload();break;}};if (window.addEventListener) {window.addEventListener("message", handleIFrameMessage, false);} else if (window.attachEvent) {window.attachEvent("onmessage", handleIFrameMessage);}</script>

    Let us know if problem persists.

    Thankyou,

    Please try to use only iFrame code, not the script tag. Just insert the following code and let us know if you still facing this issue.

    <iframe id="JotFormIFrame" onload="window.parent.scrollTo(0,0)" allowtransparency="true" src="//form.jotform.us/form/41614669045154" frameborder="0" style="width:100%; height:11426px; border:none;" scrolling="no"></iframe>

    I hope this would work. Do let us know, how it goes.

    Thankyou,

    I just turned scrolling from no to yes, it will get the job done for now. But if there is a way to make the screen go back to the top of the form after a page break, that would be super helpful and nice. Thank you for all your help!!!!
