Created a form used it for two months and now we can not get the form to display. What is wrong?

The form is still there, but the fields have gone missing. Please try checking your form's revision history. Here's how: http://www.jotform.com/help/294-How-to-View-Form-Revision-History. From there, you should be able to retrieve the lost changes. If not, please get back to us so we can check further.
Note that this kind of problem will happen in the form due to the following reasons:
a. The form is open in the form builder in multiple windows/location.
b. The form builder is close while saving is in progress.
c. Your session is timed out
