where do I go to fix this.
I have not been able to login with what I thought was our username and password

Since you are using JotForm as guest, you are only limited up to 20 submissions. You have received mail from us because your submissions is almost full and is currently 18 submissions. To fix this, you have to register an account using that email address (email@example.com) to increase your qouta up to 100th Submissions or you can also upgrade to any of our paid plans to receive more than 100 submissions per month.
