Since today the two JotForm forms no longer work on our website. The lightbox opens, but it only shows a blank page instead of the form. Nothing was changed on the forms, which worked fine for about 4 weeks. I have tried to copy the embedded code again, but that doesn't solve the problem. Could you help me out please?
On the bottom of the page, there are two links to two different JotForms. Both of them are not working any more.

Can you please check again on your website. It was already working when I checked.
I have found that the forms
were in disabled status on your My Forms page. I enabled them manually, and they are now working.
I am not sure why they were disabled, but enabling them seems to have resolve the issue.
The forms were disabled because they have Submission Limits setup on them
I suggest clearing the Submission Limit settings if you do not intend to use them.
