Thanks for your reply but I had to redo it, that's why you can see the content!
It was completely blank when I posted in the forum and I had to get it back up ASAP so I redid the form. I have no idea how I lost all of the content that was on it.

The reason why your form was blank previously was because the fields were removed based on your Revision History.
You can view, restore or revert you revisions using the Revision History tool. You can check this article to know how to view or revert/restore your revisions.
