While I receive a message that I've successfully entered my username and password and I am being redirected I am never redirected to the quiz app....I'm taken back to the same screen that asks for my username and password. Have been trying to log in this afternoon, 6/7/2015.

I currently could not reproduced the same issue when I checked.
Please make sure first that you were already logged in to your jotform account at https://www.jotform.com/login
and then open the link http://quizform.jotform.io/
here are the screenshots when I made the test
#1 logged in to my account.
#2 open Quiz App URL http://quizform.jotform.io/
#3 after Allowing the App, the Form Picker Wizards opened up
were you not able to do the same? Perhaps it could also be due to your browser's cache issue.
Try refreshing your browser also and retry.
Please let us know if issue still persist.

Yes, I've done the same thing and I'm not able to log in. Can you please let us know what is the browser you are using? By default our form builder works better with Chrome or Firefox over other browsers. Please switch to any of those browsers if that is not the case.
