I was testing your website and i like it very much but i ran into a problem i hope doesnt happen often.
My form called Aanmeldingen i imported from Formcentral and it was all good.
I asked one of my collegues to test the form.
After she submited the form i got back to the form creator page and now it is blank!
And i cant get it back through version management.
In the MY FORMS page it says it has one response but i cant find it.
Please explain how this happend because i like your system but i dont like form to blank out and responses to disapear.

I was able to see what you meant on your jotform http://form.jotformeu.com/form/51581816969369
You can fix this by reverting to a previous working saved version of the form using the Revision History feature.
user guide: -How-to-View-Form-Revision-History
Please try restoring the previous version.
Let us know if still not resolve.

Allready tried it as stated above.
There are no previous version available.

I'm sorry for the trouble caused to you.
I did check your form and I am able to replicate the issue you are having. Form does not have any form field added and it does not display any revision history as well.
I am not sure what is causing this issue. Let me report it to our back end team and we will get back to you as soon as we have any update from them.
Did you add any widget in your form? If yes, please let us know which widget you added and we will inform our back end team accordingly.

I changed nothing and no widget.
Tried to add autoresponder when i noticed the form was gone.

Its not a big deal as i imported the form again and now i dont have any problems but i would like to know that this is not a common issue and that when it ever happens it is possible to fix it. I am importing 20+ forms.

Our apologies. But our developers have just fixed this problem. It should not happen again, but if the issue persists, please let us know so we can create another ticket and forward it to our developers.

What was the issue causing the forms to disappear, Welvin?
I have the same issue. Although I was able to restore my blank form from yesterday's history entry I cannot use this in production if this is going to happen.

We believe the problem has been fixed since the last 6 hours. But, new people coming out and discovering that their forms were reset. So, it is hard to be sure if it is fixed completely.
